申請說明Application instructions：備妥資料並於2025/05/09申請截止日前繳交至勞教處基本勞作組。

獎助學門grade：日間學士班1年級(含)以上。
Daytime bachelor's degree second grade (inclusive) and above.
獎助對象recipients：修習113學年度第1學期基本勞作教育之學生。
Sustainability Practice Courses taken in the 1st Semester of the 113th Academic Year.
成績條件Grade Qualification：113學年度第1學期基本勞作教育學期成績，須通過。
Pass the Sustainability Practice course in the 1st Semester of the 113th Academic Year.

申請書撰寫注意事項application Precautions：

填寫帳戶代碼時，請確認身為申請人本人之帳戶，以免影響申請資格。Please confirm it is the applicant’s account.

資料填寫完畢後，請將申請表紙本資料印出，送繳至勞教處基本勞作組。After completing the information, please print out the application form and submit to Office of Experiential Learning.

印出紙本資料後，請在【中文姓名】欄以楷書書寫自己的姓名。Please write your name on the application form.

資料填寫完畢後，將申請表電子檔案儲存為PDF檔，寄件至ljupin0717@thu.edu.tw，信件主旨：基本勞作績優助學金申請-學號、系級、姓名。Save application form electronic file as PDF file Send to 【ljupin0717@thu.edu.tw】.Subject of the letter【基本勞作績優助學金申請】Student ID、grade、name.
其他限制條件Other conditions：
-
五大永續課程須皆出席且未缺曠。All 5 courses must be attended and no absenteeism is allowed.

須具有本校學籍之在學學生。Must be a student with student status.

申請人勞作歷程有不實情事，則取消該次申請資格。If there are any dishonesty on the application form, the application eligibility will be revoked.

非當學期基本勞作修習者不可申請。Students who are not taking the course that semester cannot apply.

給予獎助金額 Scholarship amount：每人5000元。5,000 dollars per person.

1.填寫列印申請書-學校存檔 paper application form to school for archiving.

（一）活動主辦單位：經政府立案成立之非營利組織。
（二）申請人資格限制：113-2學期修習勞作教育課程之學生。
（三）活動性質：合乎聯合國永續發展指標之服務性質活動。
（四）活動參與期間：114年1月4日至114年6月13日。
（五）認列申請期間：114年1月4日至114年6月13日17點止。
二、申請時數認列之流程：
（一）學生填具「社會實踐認列申請書」，檢附活動照片與服務時數證明，以紙本或電子郵件方式於當學期之勞作教育課程期間內送交勞教處審核。
（二）經本處審核通過即認列其社會永續時數，最高可認列3小時。如審核未通過者得於接獲未通過結果之五個工作日內補充申請書內容再次申請認列，以一次為限。
（三）第一項之申請書如附件資料，請自行下載。

(一)活動主辦單位填具「活動認列申請書」、「活動照片」與「欲申請認列時數之參與者名單」，於114年6月13日前以電子郵件方式提出申請。
(二)經本處審核通過者，認列該活動時數為勞作教育社會永續課程時數，最高可認列3小時。如審核未通過，得於接獲審核結果5個工作天內補件再次申請，以一次為限。
(三)第一項之申請書如附件資料，或至本處網頁下載，並請寄至勞教處承辦同仁信箱（ping8349@thu.edu.tw)
